| 内容記述 | 気管支喘息は比較的罹患率の高い疾患であるが、周術期の発作は不可逆的脳損傷や死亡に至ることがあるため、慎重な管理を要する疾患である。今回、われわれは喘息発作による手術中止から1か月後に行った手術で、再度喘息発作を呈した1例を経験したので報告する。患者は50歳、女性。身長141cm、体重58kg。下顎口腔前庭・口腔底狭小症に対して全身麻酔下に下顎口腔前庭・口腔底拡張術、オトガイ孔移植術が予定された。既往歴に高血圧症、気管支喘息、中程度の知的障害、骨髄異形成症、うつ病、貧血、胃潰瘍、腰椎すべり症、変形性膝関節症があった。また、患者には喫煙歴があり術前に禁煙指導を行ったが、禁煙アドヒアランス不良が疑われた。1回目の全身麻酔の喘息発作による中止から1か月待機をして臨んだ2回目の全身麻酔においても喘息発作を生じた。気道過敏性の改善のため1か月間の待機期間を設けたが、緊急性の低い本症例では待機期間を延長すべきであった。また、禁煙アドヒアランス不良が疑われる症例では、全身麻酔施行の決定をより慎重に行うべきであった。 | |||||

| 内容記述 | Bronchial asthma is a common disease with possibility of fatal accidents and irreversible encephalopathy if it occurs perioperatively. Careful management during the perioperative period for asthmatic patients is hence necessary. We report a case of an asthmatic attack during a rescheduled surgery. A 50-year-old woman (weight 58kg, height 141cm) was scheduled to undergo alveoloplasty. She had a history of hypertension, asthma, mental retardation, myelodysplastic syndrome, depression, anemia, gastric ulcer, lumbar spondylolisthesis, and knee osteoarthritis. Although she was advised cessation of smoking prior to surgery, she was suspected of smoking during the preoperative period. An asthmatic attack occurred during general anesthesia performed for the rescheduled surgery one month after cancellation of the first surgery, despite waiting one month for improvement of airway hyperresponsiveness. However, as this case was not an emergency, thus we should have postponed the operation for a longer interval. We believe that as the patient demonstrated poor adherence to smoking cessation, the operation had to be cancelled that day. |
