{"created":"2023-06-23T12:21:52.477797+00:00","id":13600,"links":{},"metadata":{"_buckets":{"deposit":"c4d74e72-6ea7-465b-85f4-0b8c8d315f9d"},"_deposit":{"created_by":21,"id":"13600","owners":[21],"pid":{"revision_id":0,"type":"depid","value":"13600"},"status":"published"},"_oai":{"id":"oai:asahi-u.repo.nii.ac.jp:00013600","sets":["46:190:414"]},"author_link":["4001","4403","4408","4406","4090"],"item_10002_biblio_info_77":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2001-06-20","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"1","bibliographicPageEnd":"114","bibliographicPageStart":"111","bibliographicVolumeNumber":"28","bibliographic_titles":[{"bibliographic_title":"岐阜歯科学会雑誌"},{"bibliographic_title":"The journal of Gifu Dental Society","bibliographic_titleLang":"en"}]}]},"item_10002_description_80":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"複雑性歯牙腫（complex odontoma）は、歯を形成する硬組織の増殖からなる腫瘍状病態で、主体はエナメル質と象牙質である。本病変は10歳代の下顎大臼歯部に好発し、大きさは母指頭大〜小鶏卵大が多いと報告されている。今回われわれは左側上顎臼歯部に発生した巨大な複雑性歯牙腫の1例を経験したので報告する。症例は、59歳の男性で、平成11年7月頃より67部に痛みを認め（これは義歯装着によるもの）耳鼻科受診、同年8月24日同部からの排膿を認めるようになった。レーザー照射を施行し経過観察を行っていたが、平成11年10月中旬頃より骨面の露出を認めたため、同年11月25日当院紹介来院した。初診時67部歯槽部に約40×25mmのドーム状の骨様硬の膨瘤が認められた。膨瘤の一部には潰瘍を呈する粘膜の欠損が存在し、そこには病変の一部と思われるものの露出を認め、それは白色の骨様の組織であり、可動性はなかった。X線およびCT所見は、左側上顎骨体部から上顎洞におよぶ22×18×33mmの塊状のX線不透過性病巣を認め、内部CT値は不均一で、全体に1000HU以上のCT値を示し、部分的にエナメル質と思われる3000HU以上のCT値を表す部位の散在が認められた。骨シンチでは、病巣部に集積が認められた。これらの検査所見から臨床診断として左側上顎骨内の歯原性あるいは骨原性の腫瘍性病変を疑い、平成12年2月17日全身麻酔下にて摘出手術を行った。皮膚切開に頬部切開法を用い、周囲の健常な組織を一部含め腫瘍摘出術を施行した。摘出物の病理組織診断は複雑性歯牙腫であった。術後3ヶ月目に義顎義歯を装着し、平成13年3月には上顎洞閉鎖術を施行し、14ヶ月経過した現在、特に症状の悪化傾向もなく経過良好である。","subitem_description_type":"Abstract"}]},"item_10002_description_81":{"attribute_name":"内容記述","attribute_value_mlt":[{"subitem_description":"Complex odontomas are fully differentiated tumors lesions histrogically composed of dentine, enamel, cementum and pulp tissue. The lesions occur in the second decade of life in the area around the mandible molar, and vary from tumb-tip sized to the size of a small egg. We report here a 59-year-old man who was referred to our hospital with an ulcer formation and exposure of an alveolar bony-like object at the maxillary molar area. Based on the imaging findings we made a diagnosis of odontogenic or osteogenic bening tumor. Tumor enucleation was performed via a cheek-spitting transbuccal approach under general anesthetic and the pathological diagnosis was complex odontoma. Three months postoperatively the patient received protease for missing teeth and bony defects at maxilla molar area. Subsequently, an operation was performed to close of the antra-oral fistula at 12 months after the first operation. At 14 months after the first operation, the patient's quality of life is
