In clinical orthodontic treatment of patients with these chief complaints, we sometimes encounter patients in whom gingivitis or periodontal disease is worsened by extraction. Our patient had gingivitis and gingival recession due to poor oral hygiene and functional disturbance, however, the intra-oral condition improved due to her cooperation. Both first premolars were extracted, and the orthodontic treatment involved a preadjusted edgewise appliance. An excellent facial profile was obtained due to dental improvement in the incisor area. Class I canine and molar occlusions, ideal overjet and overbite were established. After a 2-year retention period, the patient presented a stable
