We also aimed to evaluate the support available for family members in various family systems, depending on the experience of re-hospitalization.\nMethods: We evaluated patients and the family members of patients with chronic heart failure who have not been re-hospitalized. We also evaluated patients and family members of patients who have been re-hospitalized. For the evaluation, we used the Family Adaptability and Cohesion Scale IV (FACESKGIV-16), based on the couple-family circumplex model by Olson. We conducted a self-administered questionnaire survey of the patients and a self-administered questionnaire survey and an interview survey of the family members.\nResults: Regarding “balanced” family system, both patients with and without readmission performed items other than “defecation management” on the self-management behavior scale. There was a difference in how the patient's name and course of illness were perceived between the family of the re-hospitalized patient and that of the non-re-hospitalized patient and in the presence or absence of housework. In the “mid-range,” there was a difference between NYHA classification and self-management behavior items. The patient's illness is referred to as “heart disease.” There were differences in how the severity of the illness was perceived and whether specific support was provided. There was no “extreme type” without readmission experience.\nDiscussion: Nurses should encourage “balanced” family members to understand disease and create a forum for promoting disease understanding and coordinating support for “intermediate” family
