| 内容記述 | In adult skeletal open bite cases, when surgical orthodontic treatment is not performed, compensatory treatment by removing functional factors and tooth movement is applied. Treatment results as obtained in juvenile patients are not expected in adult cases due to periodontal tissue reaction and the limit of tooth movement amounts and range. Furthermore, to plan comprehensive treatment including restorative and prosthetic treatment, the establishment of treatment goals considering patients' living circumstances and psychological expectations of the treatment is necessary. The patient in this study was a 26-year-and-6-month-old female, who consulted our department with a chief complaint of esthetic disorders in the anterior tooth region, and was diagnosed as having skeletal open bite with the absence of the right mandibular second premolar and enamel hypoplasia. After extracting the bilateral maxillary first premolars, treatment was performed by the multi-bracket system, and anterior tooth overlap and functionally stable occlusion were obtained. The period of dynamic treatment was 19 months. Furthermore, in the second year of retention, porcelain laminate veneer crown restoration was performed in the maxillary anterior tooth region, and the patient's esthetic satisfaction was obtained. A stable occlusal relationship has been maintained for 7 years and 6 months after starting retention. | |||||

| 内容記述 | 成人における矯正治療は若年者と違い成長を期待できず,主として歯の移動による治療で組み立てることになる.そのため骨格的な問題に対しては,機能的要因の除去以外は歯の傾斜による代償的な解決を行うか,外科矯正的手段を併用しなければならない.また成人の場合,歯周組織の反応,移動量,移動範囲などの上でも限界があり若年者と同じような効果は望めないため,矯正治療単独ではなく,補綴などを考慮に入れながら口腔全体の機能回復をはかり,その個体の現状に最もよい咬合を得られるように治療方針が決定される.さらに,成人患者では,患者の生活環境と患者の心に内在する矯正治療への期待感が,治療の目標設定に大きく影響することもある.今回我々は,前歯部審美障害を主訴として来院した26歳6ヵ月の女性で,エナメル質減形成を有する骨格性開咬患者に対して,上顎左右第1小臼歯を抜歯して,マルチブラケットシステムにて19ヵ月の矯正治療を行った.その結果,治療後正中のずれはわずかに残ったものの,前歯部の被蓋を獲得し,機能的にも安定した歯列と咬合が得られた.さらに,動的治療終了後,約2年後には上顎前歯部にポーセレンラミネートベニア修復を施し審美的にも患者の満足が得られた.保定7年6ヵ月を経過した現在も良好な状態は維持されている. | |||||
