| 内容記述 | （目的）口唇裂・口蓋裂患者の矯正治療について統計資料を用いて報告する。 （研究資料）金沢医科大学病院形成外科の口唇裂・口蓋裂患者923 名（A群）、形成外科より矯正歯科に紹介を受けた口唇裂・口蓋裂患者211 名（B 群）である。 （研究方法）1．裂型分類（1）A 群をCL、UCLP、BCLP、CP の裂型別に分類し、それぞれの割合を算出した。（2）B 群もA 群と同様に分類した。2．B 群の各裂型別に以下の検討を行った。（1）上顎前突、下顎前突、切端咬合、開咬、過蓋咬合、臼歯部cross-bite、歯の形態異常など咬合異常の割合（2）反対咬合および臼歯部cross-bite の割合（3）矯正治療開始年齢を①（生後3 か月以内）②（生後4 か月―2 歳）③（6 歳―18 歳）④（18 歳以降）の4 期に分類し，裂型別の人数を算出した。 （結果および考察）1（1）CL39％、UCLP28％、BCLP11％、CP21％（2）UCLP54％、BCLP12％、CL15%、CP17%、2（1）全裂型で下顎前突が多くUCLP、BCLP は85%、CP52% であった。過蓋合はBCLP51%，UCLP32% であった。全裂型で歯の形態異常の出現率は高くCL84.9%、BCLP63.4% であった。（2）トータルクロスバイトはUCLP81%、CLP74% であった。（3）① UCLP，BCLP がいずれも50%、CL15％、CP7％。② UCLP、BCLP はいずれも40%、CL，CP はともに80％と高かった。③いずれの裂型も10％以下であった。 （結論）1．形成外科CLP38％である裂型分類は矯正歯科67% で、唇顎口蓋裂に矯正治療対象者が多いことがわかった。単独口蓋裂と唇顎裂は矯正治療を行う割合は少なかった。 2．全裂型で歯の形態異常と下顎前突の割合が多く、特にUCLP、BCLP で高かった。 3．臼歯部cross-bite はUCLP、BCLP で高く、上顎骨の狭窄を認めた。 |

| 内容記述 | The following findings were obtained from 923 cleft lip and palate patients treated at the Department of Plastic and Reconstructive Surgery and 211 patients treated at the Department of Orthodontics of Kanazawa Medical University Hospital. （1） When the rates of the treated cleft types were compared between the Departments of Plastic and Reconstructive Surgery and Orthodontics, cleft lip and palate （CLP） accounted for 38 and 68% in the Departments of Plastic and Reconstructive Surgery and Orthodontics, respectively, clarifying that many CLP patients have occlusion abnormalities indicated for orthodontic treatment. Inversely, orthodontic treatment was performed in fewer patients with cleft palate alone and cleft lip alone. （2） The incidence of abnormalities of each tooth was high in all cleft types and it was 84.9% in CL, 63.4% in BCLP, 60% in CP, and 55.5% in UCLP. （3） The incidence of mandibular protrusion was high in all cleft types and it was higher than 85% in UCLP and BCLP. （4） The incidence of posterior cross-bite was 81% in UCLP and 74% in BCLP, clarifying a narrow maxilla. （5） Many cases of UCLP and BCLP were attached with NAM and Hotz plate, initiating treatment immediately after birth. |
