| 内容記述 | 本検討では、cT1N0 およびcT2N0 の早期舌扁平上皮癌症例において、病理学的深達度（pDOI）とTumor Budding（TB）が頸部リンパ節後発転移に及ぼす影響を検討した。2015 ～ 2021 年に当院で舌部分切除術のみを行った15 例を対象とし、臨床背景および病理組織学的因子と頸部リンパ節後発転移との関連を後ろ向きに解析した。頸部リンパ節後発転移は3 例に認められ、pDOI が5mm 以上の全例で頸部リンパ節後発転移を生じ、pDOI は有意な予測因子であった（p＝0.029）、TB については、Low Grade で頸部制御率100％であったのに対し、Intermediate およびHigh Grade では66.7％であった。また、TB の個数とpDOI の間には中等度の正の相関を認めた（r＝0.602、p＝0.017）、以上より、TB はDOI と同様に頸部リンパ節後発転移を反映しうる指標であり、複数の病理学的因子を組み合わせることで予測精度の向上が期待される。 | |||||||||||||

| 内容記述 | We investigated the impact of pathological depth of invasion（pDOI）and tumor budding（TB） on late cervical lymph node metastasis in early tongue squamous cell carcinoma classified as cT1N0 or cT2N0. Fifteen patients who underwent partial glossectomy without elective neck dissection from 2015 to 2021 were retrospectively analyzed. Late cervical lymph node metastasis occurred in three cases, and all patients with a pDOI of 5 mm or greater developed metastasis, showing pDOI as a significant predictor（p＝0.029）. The cervical control rate was 100％ in Low-grade TB, whereas Intermediate- and High-grade TB both showed 66.7％. TB count moderately correlated with pDOI（r＝0.602, p＝0.017）. These results indicate that TB, similar to DOI, may reflect the risk of late cervical metastasis, and that using multiple pathological factors may improve risk assessment in early tongue cancer. | |||||||||||||
