下顎の非対称を伴う骨格性下顎前突症例は,外科的矯正治療の対象となることが多く,骨切りを適応させることで口腔硬組織,軟組織の形態的改善,発音・咀嚼などの機能的改善,顔貌形態の審美的改善を図ることが可能となる.これらの症例では下顎が非対称であるため,下顎骨切りに際してはそのセットバック量に左右差が生じ,術後長期にわたる咬合の安定や顔貌の変化の安定性に影響を与える.しかし,臼歯部の適正な咬合関係を確立することにより,矯正治療後に認められる後戻りを少なくすることができ,良好な咬合状態および顔貌を維持することができる.今回,著しい下顎の偏位を伴う骨格性下顎前突症に対して,外科的矯正治療(IVRO)を適応したところ,上下顎の不調和,オトガイ部の偏位および顔面の非対称が改善され,動的処置終了後も安定した咬合関係を獲得することができたので報告する.症例は,23歳11カ月の女性で,下顎骨の変形を伴いオトガイ部は著しく右側へ偏位しているため,外科的矯正治療の適応と判断した.術前矯正治療として,プレアジャステッド・エッジワイズ・アプライアンス(.018inch slot)を装着して上下歯列の配列を12カ月間行った後,下顎枝垂直骨切り術を施行して右側を3.5mm,左側を6.5mm後方に,正中を10mm左側に移動させた.その後,約10カ月間の術後矯正を経て保定へと移行し,保定装置には上下顎ともにラップアラウンドタイプリテーナーを使用した.保定2年6カ月時に上下顎とも保定装置の使用を終了,動的処置終了3年2カ月経過しているが,顔貌に大きな変化はみられず歯列にも後戻りはみられない. |

Surgical orthodontic treatment is applied to cases with mandibular asymmetry, in which osteotomy is performed to inprove oral hard and soft tissue morphology, speech and masticatory functions, and facial morphological esthetics. With regard to mandibular osteotomy, differences in the setback amount between the right and left sides due to mandibular asymmetry influence postoperative ling-term occlusal and facial view stability. In this study, surgical orthodontic treatment was applied to a case of skeletal mandibular protrusion with marked mandibular deviation. Maxillo-mandibular disharmony and facial asymmetry were improved, and a stable occlusal relationship was obtained. The patient was a 23-year-and-11-month-old female at the first examination. As she had mandibular bone deformity with marked right deviation of the mentum, surgical orthdontic treatment was indicated. Preadjusted edgewise appliances were inserted to arrange the maxillomandibular dentition for 12 months. At the age of 25 years and 2 month, intra-oral vertical ramus osteotomy was performed. The right side was posteriorly shifted by 3.5mm, the left side was posteriorly shifted by 6.5mm, and the midline was shifted by 10mm in the left direction. After intermaxillary fixation for approximately 3 weeks, postoperative orthodontic treatment was performed for 10 month, and then retention was performed after completing dynamic treatment. Lap-around type retainers were used in the upper and lower jaws. At the age of 29 years and 6 months, 3 years and 2 months after completing dynamic treatment, the facial view showed no marked changes without relapse of the dentition. |
