{"created":"2023-06-23T12:15:19.760758+00:00","id":5046,"links":{},"metadata":{"_buckets":{"deposit":"e2226125-fb6f-4d71-a44c-2a085f02364e"},"_deposit":{"created_by":21,"id":"5046","owners":[21],"pid":{"revision_id":0,"type":"depid","value":"5046"},"status":"published"},"_oai":{"id":"oai:asahi-u.repo.nii.ac.jp:00005046","sets":["46:190:231"]},"author_link":["8842","3913","4023","341","338","3992","345","535","4120","3843","340","8581","8580","342"],"item_10002_biblio_info_77":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2011-02-20","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"3","bibliographicPageEnd":"200","bibliographicPageStart":"195","bibliographicVolumeNumber":"37","bibliographic_titles":[{"bibliographic_title":"岐阜歯科学会雑誌"},{"bibliographic_title":"The journal of Gifu Dental Society","bibliographic_titleLang":"en"}]}]},"item_10002_description_80":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"【緒言】歯科診療所における摂食・嚥下障害者への対応は、チームアプローチも含めて重要である。しかし地域によってはその役割が十分とは言えず、摂食・嚥下リハビリテーションの普及のための問題点を検討するため岐阜県嚥下障害研究会による摂食・嚥下リハビリテーション現状調査を行ったので報告する。【方法】アンケートは2007年11月に実施した。無記名回答でファックスにて回収した。岐阜県の医療機関、高齢者施設および障害児・者施設等を対象に2,239か所にアンケート用紙を送付したところ、全体の返信数は250施設(11.2%)であった。そのうち歯科診療所は922か所で、返信数は15か所(1.6%)であった。【結果】摂食・嚥下訓練を実施している歯科診療所は3か所で、未実施は12か所であった。摂食・嚥下訓練の問題点は、往診依頼で嚥下訓練を希望する人が少ない、家族やケアスタッフの認識が低い、歯科的支援の不足があげられた。将来的に摂食・嚥下訓練を行う予定がある診療所は5か所で、ないは7か所、無記入は3か所であった。今後の摂食・嚥下リハビリテーションに対し必要な情報は、情報交換の設定、地域歯科医療機関に関する情報、定例研修会による検査や訓練法の理解・習得、患者の紹介システム、患者への啓蒙活動があげられた。【考察】今回のアンケート結果では開業歯科医の摂食・嚥下リハビリテーションへの関心は低いことが判明し、その対応に苦慮している。介護保険による口腔機能向上、要介護高齢者の歯科疾患への対応や補綴治療、口腔ケアなどを含めて、歯科診療所による対応は重要であるが十分なものではない。一方で病院、高齢者施設、在宅等での摂食・嚥下障害へのニーズは今後も増加するが、地域での歯科診療所との連携が取れていないのが現状である。そのために摂食・嚥下リハビリテーションの具体的な研修制度の実施や摂食・嚥下障害者の受け皿となる地域ネットワークの構築などが必要と考える。\n\nIntroduction: In addition to the team approach, it is important that dental offices play a role in the clinical treatment of patients with dysphagia. This role is not adequately performed in all regions. In order to study issues with promoting the spread of eating and swallowing rehabilitation, we report the results of a survey conducted by the Gifu Society for the Study of Swallowing Disorders. Method: A questionnaire was carried out during November 2007. Replies were anonymous and collected by fax. Questionnaires were sent to a total of 2,239 medical institutions, facilities for the elderly, and facilities for those with disabilities in Gifu Prefecture. There were 250 (11.2%) responses. Of the total, 922 were dental offices, with only 15 (1.6%) responding. Results: Eating and swallowing rehabilitation was carried out at three of the dental offices, but not at any of the other 12. Problems identified included the fact that few patients requested eating rehabilitation, limited awareness of rehabilitation among care staff and patients' family members, and a lack of dental support. Five dental offices intend to conduct rehabilitation starting in the near future, seven dental offices have no plans to do, and three did not answer the question. We need more information about the establishment of information networks concerning eating and swallowing rehabilitation, information regarding local dental offices, regular workshops to develop understanding and acquisition of training methods and clinical examination techniques, patient introduction systems, and initiatives for educating patients. Discussion: Judging from the results of this questionnaire, disappointingly, general dental practitioners in Gifu have minimal interest in eating and swallowing rehabilitation. While it is important that dental offices play a role in the improvement of oral functions through nursing care insurance, dental treatment for elderly people that require nursing care, and oral care, this role is not performed adequately. On the other hand, the need for eating and swallowing rehabilitation at hospitals, facilities for the elderly, and for those receiving home care is increasing. Despite this, there are currently no relationships between these facilities and local dental offices. Therefore, we need to implement training systems for eating and swallowing rehabilitation and establish networks for the treatment of
