{"created":"2023-06-23T12:15:35.779881+00:00","id":5393,"links":{},"metadata":{"_buckets":{"deposit":"8ea46418-4e68-4008-90c2-2133989cd4c5"},"_deposit":{"created_by":21,"id":"5393","owners":[21],"pid":{"revision_id":0,"type":"depid","value":"5393"},"status":"published"},"_oai":{"id":"oai:asahi-u.repo.nii.ac.jp:00005393","sets":["46:348:365"]},"author_link":["3930","3933","3931","4171","1779","3934","4172","8747"],"item_10002_biblio_info_77":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2017-06","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"1","bibliographicPageEnd":"71","bibliographicPageStart":"65","bibliographicVolumeNumber":"44","bibliographic_titles":[{"bibliographic_title":"岐阜歯科学会雑誌"},{"bibliographic_title":"The Journal of Gifu Dental Society","bibliographic_titleLang":"en"}]}]},"item_10002_description_80":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"メチシリン耐性黄色ブドウ球菌(MRSA)等のβ-ラクタム耐性ブドウ球菌は院内感染や市中感染における公衆衛生上重要な多剤耐性菌である。β-ラクタム耐性の獲得はmecA遺伝子を運ぶ可動性染色体カセットSCCmecにより媒介されている。本研究では、本学学生140人を被験者としてmecA陽性ブドウ球菌の検出を行い、青年期における分布状況の推測を行うことを目的とした。各被験者から鼻腔検体を採取し、β-ラクタム薬セフォキシチンを含む選択分離培地で培養後、検出されたβ-ラクタム耐性菌についてPCR法でmecAの有無を調べた。また被験者の鼻腔検体はブドウ球菌の選択培地でも培養され、mecA陽性ブドウ球菌がどの程度常在菌として優勢なのか推察した。結果として140人中12人(8.6%)からmecA陽性ブドウ球菌が検出された。これら被験者の中にはMRSAを鼻腔常在ブドウ球菌のメジャーポピュレーションとして、あるいはマイナーポピュレーションとして保菌している可能性のある者が含まれていた。他研究グループによる過去の調査を加味して考慮すると、青年期人口におけるmecA陽性ブドウ球菌の保菌者は1割程度の割合で存在すると考えられる。MRSA等の多剤耐性菌の市中感染は増加の一途を辿ることが警戒されており、感染拡大の防止対策を打ち立てるためにもmecA陽性ブドウ球菌の動態調査を積極的に行うことが重要であると思われる。\nβ-Lactam-resistant staphylococci, including methicillin-resistant Staphylococcus aureus (MRSA), are multi-drug resistant bacteria that cause important public health problems in nosocomial and community-acquired infections. β-Lactam resistance is achieved through the acquisition of staphylococcal cassette chromosome mec (SCCmec), a gene transfer vehicle that carries mecA. In this study, we aimed to detect the carriers of mecA-positive staphylococci among 140 students of this university and aimed to describe the dissemination of mecA-positive staphylococci during adolescence. Nasal swab specimens were collected; they were inoculated and cultured on selective agar media containing cefoxitin. Detection of mecA in β-lactam-resistant colonies was performed using polymerase chain reaction. The specimens were also inoculated and cultured on selective agar media for isolation of streptococci to evaluate the infl uence of mecA-positive staphylococci among the normal staphylococcal flora of the nasal cavity. As a result, mecA-positive staphylococci were detected in 12 subjects (8.6%). These subjects included possible MRSA carriers (i.e., those who possess MRSA as the major staphylococcal flora or the minor staphylococcal flora of their nasal cavity). Taking the observation of other studies into account, these results suggest that nearly 10% of adolescents in Japan are carriers of mecA-positive staphylococci. Since increases in community-acquired infections of multi-drug resistant bacteria, including MRSA, have been documented, it is important to actively monitor the dissemination of mecA-positive staphylococci to prevent the spread of their
