 note = {緒言：朝日大学PDI 岐阜歯科診療所は、開設以来、医療安全対策に取り組んでいる。そこで今回我々は、医療安全向上を目的として、インシデントレポートを収集し、分析を行った。
方法：調査期間は2007年6 月～2015年6 月までとした。調査項目は歯科医師および歯科衛生士の経験年数、患者の年齢、発生場所、発生月、発生曜日、発生時間、発生項目とした。
結果：インシデントの総件数は186件であった。歯科医師は経験年数が少ない者の割合が高い傾向であった。歯科衛生士は30年以上が最も多かった。患者の年齢は高齢者の数が多かった。発生場所は診療室が最も多かった。発生月は10～12月が最も多かった。発生曜日は火曜日と金曜日が多かった。発生時間は午前中に多く、午後は減少傾向であった。発生項目は患者対応、予約、X 線検査、技工が多かった。
結論：インシデントレポートの収集および分析から、インシデントの実態を把握することができ、医療安全の向上に繋がると考えられる。
Introduction: Patient safety measures have been in place at the Asahi University PDI Dental Clinic at Gifu since its establishment. We collected and analyzed incident reports in order to improve patient safety. 
Method: The investigation period was from June 2007 to June 2015. The survey covered years of experience of dentists and dental hygienists, age of patients, location, month, day of the week, time of incident, and type of incident. 
Results: A total of 186 incidents were reported. The rate of incidents tended to be higher among dentists with fewer years of experience. For dental hygienists, the rate was highest among those with 30 years of experience or more. Incidents most commonly occurred in patients of older age. The location was most often the examination room. Incidents most often occurred from October to December and on Tuesdays and Fridays. Incidents tended to occur more frequently in the morning and less frequently in the afternoon. Incidents most commonly occurred during patient correspondence, appointments, X-ray examinations, and the creation or repair of dental prostheses.
Conclusion: By collecting and analyzing incident reports, we were able to learn the actual situation of incidents and apply this knowledge to improving patient safety.},
