{"created":"2023-06-23T12:18:59.858018+00:00","id":9809,"links":{},"metadata":{"_buckets":{"deposit":"3a626e3b-bfc1-43ec-ab5f-5b9ee6cd60f8"},"_deposit":{"created_by":21,"id":"9809","owners":[21],"pid":{"revision_id":0,"type":"depid","value":"9809"},"status":"published"},"_oai":{"id":"oai:asahi-u.repo.nii.ac.jp:00009809","sets":["46:348:385"]},"author_link":["340","4023","338","8768","3992","8770","4293","4289","4120"],"item_10002_biblio_info_77":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2018-10","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"2","bibliographicPageEnd":"137","bibliographicPageStart":"129","bibliographicVolumeNumber":"45","bibliographic_titles":[{"bibliographic_title":"岐阜歯科学会雑誌"},{"bibliographic_title":"The Journal of Gifu Dental Society","bibliographic_titleLang":"en"}]}]},"item_10002_description_80":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"障害児・者の歯科治療において、適応行動が得られず全身麻酔下での治療が必要な場合がある。術前検査についての研究は多く認めるが、その有用性を検討した研究は見当たらない。今回、障害児・者の全身麻酔下歯科治療における術前・術後の臨床検査値の変動を調査し術後検査の有用性を検討した。対象は1997年1月から2015年12月まで19年間に朝日大学歯学部付属病院障害者歯科で全身麻酔下歯科治療を受けた196名とした。\n　対象を、術前後に検査値に影響を与える項目として年齢、障害、常用薬の有無、術後に影響を与える項目として、麻酔方法、治療時間、治療内容で分類した。術前・術後の臨床検査は、血液一般検査、血液生化学検査を調査した。\n　調査の結果、術後多くの基準値の範囲外の検査値を認めた。術後、有意に差を認めた項目においては治療内容や治療時間、全身麻酔薬の影響に即した反応であった。障害児・者において、障害の種類は検査値に大きな影響はなく、多くの基準値の範囲外の検査値が発現しても退院に影響を及ぼすものはほとんどなかった。術後の臨床検査値において、処置内容、手術時間、麻酔薬の影響が考えられ、障害や年齢、常用薬の有無の影響は少なかった。またこれらの基準値の範囲外の検査値が退院判断にほとんど影響を与えていないことが分かった。これらのことより術前検査の結果が良好であれば、手術時間4時間未満の歯科治療の場合は術後の診査結果に問題がなければ術後臨床検査を行う必要性は少ないと考えられた。\n Treatment under general anesthesia may be necessary in the dental treatment of people with disabilities. Many researches on preoperative examination are accepted, but there are no studies that examined guidelines and their usefulness for postoperative examination. In this study, we investigated the fluctuation of preoperative and postoperative laboratory test values for dental treatment under general anesthesia for people with disabilities and reported the usefulness of the postoperative examination. \n The subjects were 196 subjects who underwent general anesthesia under general anesthesia in 19 years from January 1997 to December 2015. Subjects were classified according to anesthesia method, operation time, treatment contents as items affecting examination values before and after surgery as a factor affecting age, disability, presence of regular medication, and postoperative items. Preoperative and postoperative laboratory tests were conducted on general blood tests and blood biochemical tests. As a result of survey, many abnormal examination values were recognized after surgery, but the incidence of abnormal values did not change so much from before the operation. Even in the items that showed significant differences aftersurgery, the response was in line with the treatment content, treatment time, and the effects of general anesthetic. In the children with disabilities, the type of disability had no significant effect on the examination value, and almost no outliers were affected even if many outliers appeared. In the preoperative and postoperative clinical laboratory values, abnormal values are recognized, but postoperative treatment contents, operation time, effects of anesthetics were considered, and influence of disability, age, presence of regular medications was small. It was also found that these abnormal values have little influence on discharge judgment. Based on these findings, if the results of the preoperative examination are good, in the case of dental\ntreatment with less than 4 hours of operation time, it is considered that there is little need to perform postoperative clinical examination unless there is a problem after surgery.
