| 内容記述 | 著しい上顎前歯の唇側傾斜と上顎歯列の狭窄、下顎骨の後退、上下顎歯列の叢生を伴ったドリコフェイシャルタイプの骨格性上顎前突の初診時年齢18歳8か月女性に対して、上顎両側第一小臼歯と下顎左側第一小臼歯および保存不可の下顎右側第一大臼歯を抜去した。Le Fort Ⅰ型骨切り術による上顎骨の反時計方向への回転と下顎枝矢状分割法による下顎骨前方移動術を用いた上下顎同時移動術を顔貌の改善を目的としてオトガイ形成術を行い、良好な咬合と顔貌の改善が得られた。保定開始から1年10か月を経過した後も、安定した咬合関係を保っており、顔貌の変化もほとんど認めず、患者の十分な満足が得られたので報告する。 | |||||

| 内容記述 | This case report describes the orthodontic treatment of a 18-year-8-month-old female with severer skeletal Class Ⅱ malocclusion of the doricofacial type and maxillary protrusion with marked labial inclination of incisors, arch constriction of maxilla, mandibular retraction, extreme crowding of upper and lower dental arch. Extraction of the maxillary first premolars on both sides, the mandibular left first premolars and the unpreservable mandibular right first molar were performed. The two-jaw surgery with counterclockwise rotation of nasal floor by Le Fort type I osteotomy, anterior mandibular movement surgery by the sagittal splitting method of the mandibular ramus. In addition, the genioplasty was performed during orthodontic retention treatment. As a result, good occlusion was obtained. After as retention period of 1 years and 10 month, the occlusion remained stable with an aesthetic facial profile and the patient was fully satisfied with the treatment results. | |||||
