値 Under the mechanical loading, cardiac fibroblasts switch to myofibroblasts which express α-smooth muscle actin (α-SMA). In fibrosing hearts, myofibroblasts are associated with cardiac extracellular matrix remodeling. It is well known that myofibroblasts are increased not only in the scar of post-infarction hearts but also in failing hearts subjected to pressure or volume overload. However, the clinical significance of the myofibroblasts in dilated cardiomyopathy (DCM) is still unclear, which we here investigated using left ventricular (LV) biopsy specimens obtained from 43 DCM patients who were diagnosed in our hospital in 2009 to 2013. Immunohistochemitry for α-SMA demonstrated the bundles of myofibroblasts localizing in the subendocardium in 23 of 43 specimens. These cells were immuno-positive also for desmin, but not for vimentin or fibronectin. Ultrastructurally, the cells contained massive actin filaments, well developed endoplasmic reticulum, and sparse glycogen granules. In the bundles-positive group, the LV peak wall stress (164.1±46.6 vs. 198.7±44.8 dynes⋅103/cm2, p<0.05) and LV end-systolic meridional wall stress (96.5±37.6 vs. 120.3±38.3 dynes⋅103/cm2, p<0.05) were significantly smaller and the ejection fraction was greater (41.1±9.7 vs. 34.6±11.1 %, p<0.05) compared with the bundles-negative group, while the age and the sex were not significantly different between the groups.These results suggest that the bundles of myofibroblasts may be one of the compensatory mechanisms against heart failure, which attenuate wall stress and cardiac remodeling.
