| 内容記述 | 本研究は、顎関節雑音症例に対して非治療による15ヵ月間の経過観察を行った。被験者は、経過観察群(10名)、顎機能健常者群(5名)、顎関節症患者群(5名)の3グループを観測対象として下顎運動および顎関節雑音とを合わせて検討した。その結果、以下の結論を得た。1.経過観察群10名中6名は経過観察期間を通して雑音が再発しなかった。他の4名中3名は経過観察中に一旦症状が悪化した。残りの1名(被験者B)は、経過観察中を通して雑音を自覚し、症状に変化がなかった。2.Kendallの一致係数は、症状におおむね対応した変動を示した。3.経過観察群で一旦症状が悪化した3名と患者群4名の雑音発生様相は共通していた。開口末期あるいは閉口末期に、明確なクリッキング波形に弱い持続性のクレピタス波形との混合型波形であった。4.その顎運動路と顎関節雑音の同時記録の分析から、顎関節の病態がある程度把握できること、また症状の再発の有無が推測できることが示唆された。 | |||||||||||||

| 内容記述 | In this study, we performed a15-month follow-up observation without treatment in patients with temporomandibularjoint sound. The subjects consisted of 10 patients with temporomandibular joint sound(follow-up observationgroup),5 subjects with healthy temporomandibular function(healthy group), and5 patients with temporomandibularjoint disorder(patient group). In these3 groups, mandibular movement and temporomandibular joint soundwere examined.The following results were obtained:1.In the follow-up obsevation group, temporomandibular joint sound did not recur in6 of the10 patients duringthe follow-up observation period, but symptoms were temporarily aggravated in3 of the remaining4 patients. Theremaining1 patient(subject B )was aware of temporomandibular joint sound during the follow-up observation period,and the symptom remained unchanged.2.kendall's coefficient of concordance corresponded to changes in the symptoms.3.A similar development pattern of temporomandibular joint sound was observed in 3 of the 4 patients in thefollow-up observation group, in whom symptoms were temporarily aggravated, and4 patients in the patient group. Itwas a mixed wave pattern of clear clicking and weak, continuous, crepitus wave patterns at the end of mouth-openingor mouth-closing.4.Analysis of the simultaneously recorded mandibular movement and temporomandibular joint sound suggested thattemporomandibular joint conditions can be estimated, and the recurrence of symptoms can be predicted. | |||||||||||||
