| 内容記述 | 小児期の機能的障害については,的確な診断・評価が必要であり,そのために基本的な成長発育の把握や正しい指標設定が重要である.そこで,小児の最大開口量と顎関節症の調査および嚥下について検討を行った.その結果,最大開口量は増齢的に増加し,男子が女子より大きく,増齢に伴い差が大きくなった.顎関節症は増齢的に増加し,高校生では女子に多く認められた.顎関節症状の経年的研究では,第二乳臼歯脱落後に第二大臼歯が萌出した児童に症状の消退が多かった.顎関節症状の継続率は中学生に比べ高校生,特に女子が高かった.嚥下時には側頭筋,咬筋筋活動が先行し,顎二腹筋,顎舌骨筋は喉頭挙上とほぼ同時に開始していた.この協調運動は,小児と成人間では違いが見られる傾向を示した.また頭位前傾位は咽頭腔形態を変化させ嚥下開始の遅延を代償し,体幹後傾位では食塊の咽頭方向への輸送を容易にし誤嚥を防止していることが明らかとなった. | |||||

| 内容記述 | We have investigated mouth-opening capacity and swallowing, and the incidence of TMD in children. The following results were obtained : 1. The mean value of the maximal mouth opening increased with age, and the value was greater in boys than in girls. 2. The incidence of TMD significantly increased with age. There were no differences based on gender from pre-school to junior high school, but females showed a significantly higher incidence than males in high school. 3. As for the continuation rate for three years of the TMD symptoms, it was higher in the high school than in the junior high school students, especially in females. 4. In the temporal relationship between laryngeal movement and orofacial muscle activities, the temporalis and masseter muscles were activated before the onset of the sudden laryngeal elevation when swallowing, whereas the onset of the digastric and myelohyoid muscle activities nearly coincided with the laryngeal elevation. 5. There was a tendency that the onset of the digastric and myelohyoid muscle activities of adults was earlier than that of children. 6. The onset of digastric and myelohyoid muscle activities at the chin-down and the extended position of the head was earlier than that at the upright position. However, the onset of digastric and myelohyoid muscle activities at the declined position of the trunk was more delayed than that at the upright position. |
