| 内容記述 | 仮着用セメントの残留は歯肉に悪影響を及ぼす可能性がある。本研究ではクラウン仮着後の仮着用セメントの残留について実験的に検討した。下顎第一大臼歯に全周シャンファーが形成された人工歯（ニッシン社，A55A162）を用い、これに直接ワックスアップを行い、12％金銀パラジウム合金（ジーシー社、キャウスウエルM.C.12）にて全部鋳造冠を製作した後、模型（ニッシン社、D16FE-500A）に装着し、実験に供した。仮着用セメントには、ポリカルボキシレート系1種（HY）、グラスポリアルケノエート系2種（FT、IP）、酸化亜鉛ユージノール系2種（TC、TP）、レジン系1種（TL）の合計6種類を用いた。説明書に従い適量を練和し装着し、余剰分を説明書に従い除去し、硬化まで待った。その後、肉眼的に仮着セメントを除去した。ただし、HYおよびIPでは仮着直後にウォータースプレーにより余剰の仮着用セメントを洗い流し、硬化後に除去した。セメント除去後にクラウンマージン部の頬側および近心側を光学顕微鏡で50倍にて観察した。頬側では、TLでセメントの残留が認められたが、その他の仮着用セメントではセメントも残留はほとんどなかった。HY、FT、TCおよびTPでは近心を観察したが、HYおよびTTではクラウン辺縁にセメントの残留はほとんどなかった。TCおよびTPではセメントの残留が認められた。レジン系の仮着用セメント、酸化亜鉛ユージノール系の仮着用セメントは、歯肉溝内に残留する可能性が高いことが判明した。 Cement residue after temporary cementation of crown may induce harmful eff ects on gingival tissue. In this study, we experimentally investigated the cement residue following temporary cementation of crown. Wax up of full cast crown was performed directly on an artificial tooth to which chamfer was formed around the entire periphery of the mandibular first molar. The full cast crown was prepared by casting the wax crown with 12% gold-silver-palladium alloy (GC，Castwell M.C.12) and polishing it at the end. The full cast crown was fixed on the model and used in the experiment. Six types of cement were used in crown cementation: 1 type of polycarboxylate cement (SHOFU INC., Hybond temporary cement;HY) 2 types of glass polyalkenoate cement (GC Corp., Fuji TEMP;FT and SHOFU INC., IP Temp cement;IP), 2 types of zinc oxide eugenol cement (Waterpic, OPOTOW trial cement; TC and OPOTOW temporary cement; TP) and 1 resin cement (DENTAX GmbH & Co, templink clear;TL) An appropriate amount of the cement was mixed and placed, and then the excessive portion was removed following the manual. Once the cement became hardened, it was removed with the naked eyes. The HY and IP cement however were irrigated with water spray immediately after cementation, and the excessive portion of the hardened cement was removed. Then, the crown margin of buccal and proximal side were observed at x50 with a light microscope. There was little cement residue, except for the TL, on the crowns on the buccal side. The TL was the most commonly observed as residue. Cement residue in proximal side of HY, FT, TC and TP were also observed. There was hardly any HY and TF left on the crown margin. Cement residue was observed with TC and TP. It was revealed that the resin and zinc oxide eugenol cement used for temporary cementation of crown are likely to remain inside the gingival sulcus as residue. |
