| 内容記述 | Ⅰ．目的 わが国は超高齢社会を迎え、要介護高齢者の増加とともに社会的環境も変化し、要介護高齢者への訪問歯科診療は今後、需要が増加すると考えられる。朝日大学PDI岐阜歯科診療所では、2012年11月から訪問歯科診療を実施しており、2015年6月までの2年8か月間に182名の患者の診療を行った。今回は、このうち65歳以上の訪問歯科診療の概要、問題点、今後の課題ならびに歯科治療へのニーズを検討した。 Ⅱ．対象と方法 対象者は、65歳以上の171名とした。調査項目は、性別、年齢、診療場所（居宅・施設）、主訴、治療内容、基礎疾患、要介護度、歯科治療時の患者状況、義歯の使用状況について、診療録に基づいて検討を行った。 Ⅲ．結果と考察 調査対象者は男性57名、女性114名であり、最高齢者は99歳女性、最年少者は66歳男性であった。平均年齢は83.8歳で、75歳以上の対象者が全体の約9割を占めていた。診療場所は、居宅が40名、施設は131名であった。主訴は、義歯に関するものが約半数を占め、治療内容は117名（68.4%）が義歯製作、義歯調整などの義歯による補綴治療を行っていた。対象者171名は全員何らかの基礎疾患を有しており，要介護認定者は156名いた。歯科診療時の患者状況では坐位の保持、意思疎通、含嗽ともに8割以上が可能であった。義歯を使用している者は、124名（72.5%）であった。厚生労働省では2015年から「在宅医療・介護連携推進事業」を進めており、今後高齢者を対象とした施設が増えるだろう。訪問歯科診療は、義歯などの歯科治療だけではなく、専門的口腔ケアや摂食嚥下リハビリテーション等の機能訓練が増加すると考えられる。 I. Introduction Japan has faced a super aging society and its environment has changed with the increase in number of elderly people who need long-term care and homebound dental care. It is expected that the needs of homebound dental care for the elderly people will be further increased in the near future. In the Asahi University PDI Dental Clinic at Gifu, department of homebound dentistry has been established since November 2012, and 182 patients received homebound dental treatment during 20 months until June 2015. Among them, we investigated the outlines, problems, future tasks and needs for over 65 years old patients in this study. II. Materials and Methods The subjects were 171 patients aged 65 or older. On the intraoral examination and the dental record, we examined about gender, age, dental treatment place (in-home / institute), main complaint, treatment details, underlying disease, degree of need for care, patient condition at treatment, and denture use situation. III. Results and Discussion Of the 171 subjects, 57 were male and 114 were female. The highest age subject was a 99-year-old woman, and the lowest age was a 66-year-old man. The average age was 83.8 years old, and about 90% of the subjects were over 75 years old. Regarding the place of dental treatment, there were 40 subjects at home and 131 subjects in the facility. The main complaints were mostly related to dentures, and 117 subjects (68.4%) received prosthetic treatment with dental prostheses such as new denture made and denture adjustment. Almost all 171 subjects had underlying diseases and 156 subjects were certified as requiring nursing care. In the patient condition at the time of dental treatment, however, more than 80% of the patients were able to retain sitting position, communicate and wash the mouth, and the percentage of denture users was as high as 72.5%. Since 2015, the Ministry of Health, Labor and Welfare promotes “Long-Term Care and Nursing Care Cooperation Promotion Project” and the number of nursing home for elderly people will further increase in the future. It is likely that homebound dental care, not only denture treatment but also functional training such as professional oral health care and eating swallowing rehabilitation, will be focused on more and more. |
