| 内容記述 | 朝日大学医科歯科医療センターは摂食嚥下外来の専門医療機関として、評価指導を行っている。今回、摂食嚥下障害患者への院内システムや支援方法を充実させることを目的に、当センターにおける摂食嚥下外来の実態を調査し検討した。 対象は2003年摂食嚥下外来開設から2020年までの18年間に摂食嚥下障害を主訴に受診した新患患者835名とした。調査方法は診療録をもとに、後方視的に調査を行った。年間平均初診患者数は46.4±16.4名であった。紹介元は一般医院・総合病院が31.0%と最も多く、次いで障害者支援施設が24.2%であった。初診時のライフステージ毎の患者分布では老年期（65歳以上）の受診が32.6%と最も多く、次いで幼児期前期（1～3歳）の16.3%であった。基礎疾患の内訳では、神経系疾患が35.3％と最も多く、次いで精神および行動の障害が18.7%、循環器系疾患が17.2%あった。嚥下精密検査 の実施は、全患者数の87.5%あった。そのうち、嚥下内視鏡検査は62.3%、嚥下造影検査は25.3%あった。当センターは医療機関や施設からの紹介患者が多く、発達期や老年期の多様な基礎疾患の摂食嚥下障害患者に対応していた。嚥下精密検査により精度の高い摂食嚥下機能評価と食形態や介助方法など具体的な指導方法の構築に役立っていた。多面的に診断、評価を行い、療育や介護の現場それぞれと情報共有を行って連携を深めていくことが重要であった。 |

| 内容記述 | From 2003 to today, Asahi University Medical and Dental Center was providing evaluation and advise as a specialized medical institution for dysphagia. We examined the actual condition of patients with dysphagia at our Medical and Dental Center for the purpose of enhancing the clinic system and support methods for patients with dysphagia. The subjects were 835 patients with dysphagia from 2003 to 2020. The survey method used medical records. The average number of patients for the year from 2003 to 2020 was 46.4±16.4. The most common referral sources were general clinics and general hospitals at 31.0%. Support facilities for persons with disabilities were the second most common at 24.2%. According to age brackets, in early infancy was the second highest at 16.3%. Among the underlying disorders, nervous system disorders were the most common at 35.3%, followed by mental and behavioral disorders at 18.7% and cardiovascular disorders at 17.2%. The number of detailed swallowing examination was 87.5%. Swallowing videoendoscopic was 62.3% and swallowing video fluorography was 25.3%. As a result of survey of patients with dysphagia at our center, there were many referrals from medical departments and support facilities for persons with disabilities, and the proportion of them in early infants and old age was high. By detailed swallowing examination and the expertise of the oral cavity, it was possible to provide specialized evaluation and guidance, especially regarding dysphagia in the early stages of development and old age. To increase the number of patients who can take oral intake safely, it is necessary to further enhance the medical care system and to provide information, cooperate with each site of nursing care and long-term care from the viewpoint of life support and follow up. | |||||||||
