| 内容記述 | ICUに所属する看護師がICUで認識した倫理的問題と倫理的問題への対応を明らかにすることを目的に、ICUでの勤務経験が1年以上ある看護師5名を対象として半構造化面接調査を実施した。調査結果は質的記述的に分析し、167の要約から18サブカテゴリ、6カテゴリを生成した。生成したカテゴリは【重症のため 患者の意思を尊重して看護できない葛藤】、【治療優先のICU環境への疑問】、【疎通を図ることが難しい患者の状態や治療上の制限を踏まえたケアの工夫】、【重症患者を支援する家族への支援】、【ICU退室後を意識】、【患者の病状が変化しやすいICUにおける看護師間、医師－看護師間の方針統一】であった。ICUに所属する看護師らは【重症のため患者の意思を尊重して看護できない葛藤】を倫理的問題として認識し、対応として【疎通を図ることが難しい患者の状態や治療上の制限を踏まえたケアの工夫】と【重症患者の家族への支援】、【患者の病状が変化しやすいICUにおける看護師間、医師－看護師間の方針統一】を実践していた。さらに、ICUに所属する看護師らは【治療優先のICU環境への疑問】を倫理的問題として捉え、対応としてICU環境を変えるのではなく、【ICU退室後を意識】して患者を支援していた。 |

| 内容記述 | The purpose of this study was to clarify the ethical issues recognized by nurses in the ICU and their correspondences. We conducted a semi-structured interview survey of five nurses who have worked in the ICU for more than one year. The survey results were analyzed qualitatively and descriptively, and 18 subcategories and 6 categories were generated from 167 summaries. The generated categories were [Conflict that the patient is severe and cannot care for the patient’s intention], [Doubt about the ICU environment that prioritizes treatment], [Ingenuity of care based on the condition of patients who have difficulty in communicating and treatment restrictions], [Support for the family supporting severe patients], [Awareness of progress after leaving the ICU], and [Unification of policies between nurses and doctors/nurses in the ICU where the patient’s condition is liable to change]. The nurses belonging to the ICU recognized [Conflict that the patient is severe and cannot care for the patient’s intention] as an ethical issue. As correspondences, they practiced [Ingenuity of care based on the condition of patients who have difficulty in communicating and treatment restrictions], [Support for the family supporting severe patients] and [Unification of policies between nurses and doctors / nurses in the ICU where the patient’s condition is liable to change]. The nurses who belong to the ICU regard [Doubt about the ICU environment that prioritizes treatment] as an ethical issue, and instead of changing the ICU environment as correspondences, they support patients with [Awareness of progress after leaving the ICU]. | |||||
