| 内容記述 | 下顎運動は下顎頭の回転運動と滑走運動の組み合わせであり、その発現様相は下顎機能上重要な意味を持っている。一方、咬頭嵌合位あるいはその付近における咬合接触の異常は、顎機能異常を発症させる寄与因子であり、習慣性閉口位と咬頭嵌合位との間の下顎変位の量的要素は、顎機能を評価する指標になり得ることが示唆されている。本研究は軽度咬みしめ(LCP)と中等度咬みしめ(咬頭嵌合位、ICP)の両下顎位における咬合接触面積のAsymmetry Index(AI)と習慣性下顎開・閉口時の下顎頭動態の左右差との間の相互依存関係を推察し、検討を行った。咬頭嵌合に伴う下顎の変位に対しては、僅かであれば上下顎歯の歯根膜、顎骨自体の変形でおおむね補償するが、閾値を超えると顎関節周囲組織に影響を及ぼし、左右の下顎頭に機能的なアンバランスを生じさせることを示唆するものである。 | |||||||||

| 内容記述 | Mandibular movement involves a combination of rotary and gliding movements of the caput mandibulae, and the conditions of such movements significantly affect the mandibular functions. Abnormalities in occlusal contact at either the intercuspal position or its adjacent positions is a contributing factor to the development of jaw dysfunction, and evidence suggests that the quantitative factors of mandibular displacement between a habitual closing position and an intercuspal position can be used as an index for assessing the jaw function. In this study, we examined the interdependent relationship between the asymmetry indexes (AI) of an area of occlusal contact at the mandibular positions during both light clenching (LCP) and intermediate clenching (intercuspal position, or ICP) and the differences in asymmetry in the dynamic state of the caput mandibulae during habitual mandibular opening and closure. Slight mandibular displacements with intercuspation are mostly compensated by the displacement of the periodontal ligament of the upper and lower teeth and the jawbone, but exceeding the threshold value affects the tissues surrounding the temporomandibular joint, thus indicating a possible cause for the functional imbalance in the right and left caput mandibulae. | |||||||||
